Mission Statement

Heart 'n Home Foundation

Established in 2004, Heart 'n Home Foundation is a registered 501(c)(3) non-profit organization. Heart 'n Home Foundation exists to support Heart 'n Home Hospice & Palliative Care in its mission of providing emotional, physical, and spiritual support to our patients and those who love and care for them. The Foundation's purpose is to be of financial assistance to terminally ill patients and their families for whom other resources are limited, which is made possible through generous donations from the community.

Hospice care is founded upon the idea that everyone has the right to live with dignity and without pain throughout their life. Hospice professionals and volunteers contribute to the quality of life in America at an unparalleled level in today's society. According to the National Hospice and Palliative Care Organization, there were nearly 950,000 Americans that utilized hospice care in 2003, enabling them to live out their lives free of pain with dignity and respect. Additionally, most of these patients were able to spend that time in their homes or in a home-like setting, surrounded by their loved ones rather than alone in a hospital.

Heart 'n Home Foundation is governed by a board that approves and disperses funds based upon written application from Hospice staff. As needs arise, it is the duty of the Foundation to ensure that all Heart 'n Home patients have the necessary resources available, regardless of financial ability. As the need for hospice care grows, Heart 'n Home is dedicated to providing excellent end-of-life care and ensuring community-wide access.
