Heart ‘n Home
Hospice & Palliative Care provides each of its patients with
RN visits on both a regular and as needed basis. On each visit,
the nurse assesses patient and family needs as well as dialogues
and works with the patient's physician, hospice medical directors,
and members of our interdisciplinary team to manage the patient's
medical issues and other symptoms. There are also nurses on-call
24 hours a day who are available to provide guidance, answer questions,
and visit, as necessary. The role of Heart ‘n Homes nurses
is to educate and guide patients and families throughout the illness
and to provide information needed to understand, cope, and provide
care for their loved ones.
